Patients with multiple sclerosis (MS) are often separated from clinics and hospitals by distance and neurological disability making coordination of care challenging. Telemedicine tools are proposed as a means to improve coordination and care access to VHA services. Unfortunately, many current telemedicine tools have not been effective in meeting the needs of MS patients or providers. This lecture will introduce the web-based Home Automated Telemanagement (HAT) system being designed for MS. MS HAT is based on Wagner's model of chronic disease care and provides support for patient self-management, comprehensive patient-provider communication, and multidisciplinary care coordination.
